掌超家用：居家动态监测胸水第一例-(20221129)

(一)背景

患者肖绍博(居家管护代号BXS0406)，男，78岁，2022年4月在北京某三甲医院经病理会诊确诊为“恶性胸膜间皮瘤”后，出院居家谋求/实施中西医结合综合康养最适方案，请北京慧家健康信息力研究院(BHI)助力。

该患者可定量检测/监测的主要临床异常是胸部CT或B超显示“左侧大量胸水”，相关临床表现主要是干咳、气短、胸胁痛。出于安全性考虑，患者选用B超为日常监测手段。

到医院做超声，需要完成预约挂号缴费等手续，加上路途和等候时间，就近完成一次胸水检查，至少需要半天时间，而技师完成这样的超声检查仅需10-15分钟。

(二)居家试用掌超的探索

2019年初，在全全健康公益项目学术讲座上，超声专家委员会主任毓教授介绍了“掌超在国内外研发和应用最新动态”。在了解了老友上述病情后，毓主任告知，可以帮助联系厂家，多方合作探索居家试用掌超监测胸水，患者欣然同意。在厂家、专家和BHI健康管护师共同努力下，以患者BXS0406为中心的，居家掌超监测胸水探索开始了。这也是全全公益项目的一部分【附注】。

(三)试用效果与新发现

经本人反复摸索学习，专家面对面示范和远程指导并在门诊就医时请教医院技师，该患者家庭相信，已初步掌握了掌超测胸水的基本知识和操作技巧，可初步满足在家实时检测监测胸水(参见附图)的需要。

主要发现之一，是居家掌超结果与医院大型超声设备结果大体一致，但方便得多，可以及时发现新变化，有利于巩固疗效，指导治疗和康复方案的优化。

主要发现之二，是患者观察到胸水逐渐下降时，恰逢患者暂停和减半再减半中医处方抗癌药物剂量，同时大量增加体育锻炼康养内容。在本例康养/治疗措施和胸水之间，一定存在定量或半定量关系，相信掌超将在这样的探索研究之中发挥重大作用。
